Minor Passport Application Requirements.
First-Time Applicant (Minors):
Applicants must submit the following documents:
- A Completed passport application form
- Two (2) identical non digitized color photographs one of which should be certified by the Notary Public who signs Section G
- A Original Birth Certificate
- Letter of Certificate of Citizenship, if born abroad
- Original Adoption Register, where applicable
- Parent's ID/Guardian's ID (along with letter of guardianship for the latter)
Renewal of Passport/Damaged Passport (Minors):
Applicants must submit the following documents:
- A Completed passport application form
- Two (2) identical non digitized color photographs one of which should be certified by the Notary Public who signs Section G
- The Expired Passport
- A Original Birth Certificate,
- Letter of Certificate of Citizenship, if born abroad
- Original Adoption Register, where applicable
- Parent's ID/Legal Guardian's ID (along with letter of guardianship for the latter)
Lost/Stolen Passport (Minors):
Applicants must submit the following documents:
- A Completed passport application form
- Two (2) identical non digitized color photographs one of which should be certified by the Notary Public who signs Section G
- A Original Birth Certificate
- Letter of Certificate of Citizenship, if born abroad
- Original Adoption Register, where applicable
- Parent's ID/Legal Guardian's ID (along with letter of guardianship for the latter)
- Police Complaint Report
