Adult Passport Application Requirements.

First-Time Applicant (Adults):

NOTE: All documents are to be submitted as originals which will be returned to applicant.

Birth, Marriage and Death Certificates from Jamaica are to be in the NEW FORMAT issued by the Registrar General's Department (www.rgd.gov.jm)

All first time applicants must visit the Consulate General to be interviewed by a Consular Officer and submit the following documents:

  • A Completed passport application form
  • Two (2) identical non digitized color photographs one of which should be certified by the Notary Public who signs Section G
  • A Original Birth Certificate (NEW FORMAT)
  • Original Marriage Certificate, where applicable
  • Original Adoption Register, where applicable
  • Divorce Judgment, if applicable

If Born abroad, in addition to the Birth Certificate, please submit ONE of the following: for First Time / Renewal / Damaged / Lost or Stolen Passport.

  • Certificate of Naturalization (Foreign Country)
  • Certificate of Citizenship (Jamaica)
  • Certificate of Registration (Jamaica)

In addition, ONE of the following must be presented:

  • A Driver's License ( Jamaican or Foreign Country of residence)
  • A Valid Photo Identification
  • A National Voter's Identification Card
  • A Work Permit from the country of residence
  • A Passport from country of Citizenship or Residence
  • A Permanent residence Identification card
  • Any other document (s) requested by the Consular Officer

Renewal of Passport/Damaged Passport (Adults):

Applicants must submit the following documents:

  • A Completed passport application form
  • Two (2) identical non digitized color photographs one of which should be certified by the Notary Public who signs Section G
  • Applicants who were issued passports as a minor must submit a valid Government issued photo identification.
  • The Expired Passport
  • A Original Birth Certificate (NEW FORMAT)
  • Original Marriage Certificate, where applicable
  • Original Death Certificate, where applicable
  • Original Adoption Register, where applicable
  • Divorce Judgment, where applicable
  • Any other document (s) requested by the Consular Officer

Lost/Stolen Passport (Adults):

Applicants must submit the following documents:

  • A Completed passport application form
  • Two (2) identical non digitized color photographs one of which should be certified by the Notary Public who signs Section G
  • A Original Birth Certificate (NEW FORMAT)
  • Original Marriage Certificate, where applicable
  • Original Death Certificate, where applicable
  • Original Adoption Register, where applicable
  • Divorce Judgment, where applicable
  • A Police Complaint Report
  • Any other document (s) requested by the Consular Officer
